This form should be used after termination of employment to request a refund of employee contributions by a retiree who returned to work in a certified area of a teacher shortage under the provisions of La. R.S. 11:708 or who participated in the Deferred Retirement Option Plan (DROP) under the provisions of La. R.S. 11:791. Section I must be completed by the retiree and forwarded to the employer. Section II must be completed by the employer and submitted to Teachers’ Retirement System of Louisiana (TRSL) immediately after termination of employment. Refunds will be made only after all contribution reports have been received by TRSL.
